He wants to make sure that they're not just friends, but "special ones" as well. He tells the doctor that he just called for the money, and that's why the doctor is crying. The doctor says that they can be friends if they don't need to be friends. The narrator says that if it weren't for money, he wouldn't be crying like this. He says that the reason why he wanted to be kind to the doctor was because he needed money now. He asks the doctor where he is going, and the doctor tells him that his mother will be worried about them if they do not come home. He also says that he can give the doctor the money so that he won't have another friend to cry on. He adds that the doctor's smile is as good as ever, but it's because the doctor doesn't seem to be smiling anymore. He then tells the story of how he has been living a terrible life, and how he needs to make up for all the mistakes he has made and donate his organs to people who need them.
