He says that he was sick when they were in college and that they didn't really know each other back then. So how could he be his "incentive onset" of paranoid schizophrenia? The doctor doesn't want to tell him because he'd tear him apart if he found out. He also tells him not to wet the wound and not to tell the doctor that he came to see him. He wants to know if the doctor can get his clothes for him and then bring him back to feed him. Then he asks where he can find his mouth. He points out that there are three rooms in the building and that there's a locked room on the first and a second floor. Maybe the doctor is trying to hide something. The song ends with the doctor asking if they knew each other before.
