He says that he does not know what he is putting in his mouth at the moment and that it is fine for him to eat more than he is allowed to. He recalls a time when he had to eat his lunch with his own hands and that the patient who was suffering from a disease swallowed it on his own. The patient lost his ability to breathe and cough, and he stopped being able to do other things. The doctor says that this is why he has to make sure that he is able to save the patient.
