The doctor and his wife are talking about their love for each other. The doctor tells his wife that he fell in love "with 100 percent wild and open sexual" but that it's only when we're in love that we move on. He tells her about the midwife who used to work at the midwife's house, but that she stopped working when the midwife shut down the hospital. He also tells her that the woman's health has started to break down, too, and that she's paid for all of the hard work that she put into the hospital now. He's happy to hear that the baby is healthy, but he doesn't think that the sickness is a "remedy" for it. He says that it isn't like she has dementia or senile dementia. He thanks his grandfather for taking care of him, and says that he hopes that he'll watch over the baby and give her strength. He adds that he wishes that everyone on the island were as healthy as he is, so that they wouldn't have to work so hard to keep up appearances.
