The narrator explains that the hospital is thirty floors long, and there are eighteen floors of warehouses and special purpose rooms. The only thing that's different is that there's a secret room on the 18th floor, which leads to the factory floor outside. They can't find it on the map, so they have to guess where it is by guessing where on the floor they'll find it.
