Two patients in a car accident require immediate head surgery. Dr. Bai is unable to perform the surgery because an elderly patient with a brain stem infarction and a severe respiratory failure is not suited for thrombolysis, a blood thinner that is used to treat patients with severe brain injuries. The younger brother, Dr. Xue yun, is also unavailable for the surgery. The elder brother, who is an eight-time top student at the medical school, is not pleased with his brother's refusal to perform surgery. He tells his sister that even if they have a person who can perform brain surgery, they still have to find another person for help. He remembers a patient who had a brain injury five years ago and who was saved by the younger brother. The older brother, however, knows that the patient cannot be saved. He does not think that Dr. bai did anything wrong, as he had saved the patient five years earlier. The sister asks her brother if he chooses to save the patient who is the most seriously injured or to save as many people as possible. She asks him to raise the patient.
