In this chapter, we learn that ikoma has returned to Japan to work as a project manager for a company in the United States. She has just finished an interview for a new job at a third company in Japan, and she is very excited about the opportunity. She tells us that she has been suffering from a painful stomach pain, and asks the doctor to send an ambulance. The doctor tells her that her surgery is the next day, and that she can be discharged the following week. She is shocked to see that he is actually her doctor, and tells him that he has been looking forward to becoming a doctor for a long time. He tells her to stop using the nickname "doctor" and to stop referring to him as "my doctor" because it sounds like he's unhappy about his job. He says that he isn't his doctor, but that someone else has been asked to perform the surgery for him. The next morning, the doctor tells him to shave his pubic hair and prepare for the surgery.
